Release of Liability Waiver + Medical Release Form

I give my registered camper(s), permission to participate in all activities at Winamac Youth Camp, July 23-26, 2026, unless otherwise communicated with Youth Camp Staff prior to Camp.

I release and hold harmless Hutter Construction, and its employees and all Camp Winamac Staff and their affiliated churches from any and all actions of any nature of personal injury arising from my child’s participation and stay at Camp Winamac.

I therefore, grant permission for First Aide to be administrated, proper treatment and/or hospitalization to be

secured for my child in case of emergency, according to the best judgment of Camp Winamac Youth Camp Staff and volunteers.

By registering for camp, you are agreeing to all the terms above.
